Applicant's name, father's/husband's name, permanent address, present address, educational qualification, date of birth, nationality and experience (if any) in hand written application and 3 (three) copies of recent passport size attested photographs, Chairman of Union Parishad / Application form should be sent within the stipulated time along with attested copy of Civil Certificate issued by Ward Commissioner, Character Certificate issued by 1st Class Gazetted Officer and Educational Qualification Certificate. 2.
